To apply for PHISC membership, please complete the membership form. An invoice will then be forwarded to you by the PHISC Finances for payment of membership fees. If you wish to make payment immediately, please send proof of payment along with this form and an invoice to be forwarded to you for your records. If you would prefer not to complete the form online, please contact us.
Please note that you have to be a fully paid up member of PHISC before you could gain access to working documents, supporting documents and minutes.
